Abstract
This qualitative phenomenological study explores the lived experiences of nurses working in free- standing dialysis facilities in La Union, Philippines, through the lens of Herzberg’s Two- Factor Theory of motivation. With the growth of outpatient dialysis centers, understanding factors that affect nurses’ job satisfaction and dissatisfaction is vital for improving work conditions and patient care outcomes. Purposive sampling: ten registered nurses participated in semi-structured interviews. Data were analyzed using Colaizzi’s method to extract significant themes. Findings reveal various hygiene factors contributing to job dissatisfaction, including limited leave benefits, low compensation, lack of career growth, absence of on-site physicians, and strained working relationships. These factors often led to burnout and emotional exhaustion. Conversely, motivators such as proximity to home, non-toxic work environment, perks, autonomy in facility management, and access to training enhanced job satisfaction and motivation. Nurses also expressed fears regarding patient mortality and infection risks, amplified by inadequate medical supervision and safety infrastructure. Participants’ reflections varied, with some recommending free-standing dialysis facilities as suitable starting points for new nurses, while others cited concerns about professional stagnation. Study highlights the need for structural reforms addressing both hygiene factors and motivators. Recommendations include revising leave policies, improving compensation, establishing career development pathways, ensuring medical supervision, and fostering supportive leadership. Incorporating Herzberg’s theory helps clarify the balance needed between addressing dissatisfaction and promoting motivation. This research contributes valuable insights into dialysis nurses’ experiences in outpatient settings in the Philippines, offering guidance for healthcare managers and policymakers to enhance nurse well-being, retention, and quality of care.
