The Influence of Personality Disorders on Risk Assessment Tool Performance in Sexual Offender Populations: A Meta-Analytic Review

Abstract

Actuarial and structured professional judgment (SPJ) risk assessment instruments are widely utilized within criminal justice and forensic mental health systems to evaluate sexual recidivism risk. However, the pervasive comorbidity of personality disorders—particularly Cluster B disorders and psychopathic traits—raises critical questions regarding potential moderation effects on instrument accuracy. This meta-analysis synthesized empirical findings across 42 independent studies involving 16,845 individuals convicted of sexual offenses to evaluate whether Axis II personality pathology moderates the predictive validity of leading risk tools (including Static-99R, Risk Matrix 2000, SVR-20, and RSVP). Random-effects meta-analytic models revealed that risk assessment instruments maintain moderate-to-large discrimination overall (pooled Area Under the Curve [AUC] = 0.71, 95% CI [0.68, 0.74]). However, meta-regression analyses indicated that high sample prevalence of Antisocial Personality Disorder (ASPD) and psychopathy significantly attenuated the discriminative performance of purely static actuarial instruments (AUC decline of ~0.06 to 0.08), whereas SPJ frameworks demonstrated greater resilience against predictive attenuation. These findings indicate that while risk appraisal tools remain psychometrically valid across forensic populations, severe characterological pathology introduces unmodeled dynamic variance that necessitates multimodal, dynamic assessment strategies in legal decision-making.
